Schedule: Monday–Friday + On-Call Rotation
Setting: Home Hospice
Location: Porter, LaPorte, Jasper, Starke, Newton, and Lake counties
About the Role
We are hiring a Hospice Evaluation LPN to join our home-based hospice team. This position is ideal for an LPN who is confident in the field, comfortable working independently, and skilled at supporting families during the first steps of hospice care.
You will serve as our rapid-response clinician , completing timely evaluations for new referrals and guiding families through consents and admission paperwork.
